questions and answers will help
familiarize
you with Swanson’s procedures. If you have

Do I need to bring my
prescription to my appointment?
Absolutely! Your prescription allows us
to immediately begin our work. Our
patients have found that it is best to do
this two weeks before the appointment.
What if I have
to cancel my appointment?
We understand that emergencies do arise
and that appointments need to be
cancelled. We request that you
give us 24 hours notice in order that we
may call another patient who needs to get
in as soon as possible.

What if I don't
have any insurance?
It is best to discuss this issue in
advance so we can make the appropriate
arrangements. If we don't make any
prior arrangements, payment is to be
remitted at the time of service.
What is your
warranty policy?
The warranty period for custom orthoses
and prostheses is three months for
workmanship and materials. Normal
adjustments will be made at the
discretion of the practitioner at no
charge for a three month period.
Swanson Orthotic & Prosthetic Center,
Inc. cannot be responsible for
physiological or anatomical changes or
changes in patients medical
condition. There will be a separate
charge for adjustments due to
physiological or anatomical changes or
changes in a patients medical
condition. Prescribed additions of
componentry, straps, lifts, supplies,
etc., will incur a charge. There
will be a separate charge for adjustments
or repairs that are made as a result of
abuse, or rough wear, such as may occur
from sports, vocational, or unusual
activities.
Except
in certain specific instances, health
insurance vendors have no contractual
obligation to Swanson Orthotic &
Prosthetic Center, Inc. Although we
will exhaust ever effort to assist the
patient in securing reimbursement for
professional services provided, ultimate
responsibility for full payment of any
outstanding fees remains with the
patient. Infrequent or non-use of a
prostheses or orthoses does not absolve
the patient from responsibility for full
payment or prescribed or requested
professional services rendered.
Since orthoses and prosthese are
prescribed at the direction of the
physician, and custom fabricated for the
anatomy and medical condition of each
individual, they cannot be returned for
credit or refund.
It is in
your best interest to communicate with
your practitioner on a timely basis and
to allow us to resolve any problems you
are experiencing as efficiently and
quickly as possible. We will make
every attempt to meet your needs.

there is a question or concern that your
practitioner cannot resolve for you.
